We would consider offering this post as a development opportunity for the right candidate who may be unsure about applying for a Band 6 role. We would also be willing to consider part-time and job share opportunities.
If successful you will be part of a multi-professional team working collaboratively with others to deliver person-centred, coordinated care from the point of admission onto the intermediate care unit, to discharge. You will work with patients with a variety of conditions including trauma and orthopaedic patients, neurological impairment (stroke, MS, Parkinson’s, spinal injuries), frailty and those with cognitive deficits, management of long term conditions including cardiac and respiratory conditions.
Opportunities within this role include:
- Undertaking multifactorial assessments and treatment plans for patients with a variety of health conditions and backgrounds.
- Implementing individualised rehabilitation and exercise programmes.
- The challenge of improving the physical and functional abilities of someone suffering with a long term condition such as parkinsons or COPD
- Co-ordination of therapy team on a daily basis to ensure the caseload across the wards is delegated and completed appropriately
- Contributing to embedding a Discharge to Assess model
- Supervising junior staff and assistants in their development and progression within their roles.
- Service development and teaching opportunities within the established teaching programme and Quality Improvement initiatives.
